This session will outline the importance of using data to drive best clinical practice and quality improvements for service lines. Pamela and Stephanie will share strategies for developing real-time data collection that in turn supports real-time process improvement, leveraging data in the management of discrete patient populations, and collecting information once and using it many times to support efficient outcomes management and quality reporting. Further, in the context of integrating IT information management into workflow redesign, the speaker will address how to use data as a means to force new and consistent practice standards, leading to improved outcomes. A case study of a large southern health system will be used to highlight a successful process of generating data and using reporting as a means for initiating future change through IT linkages throughout the care continuum.
